If using this insurance benefit, you will be responsible for the session fee at the time of service and I will provide you a receipt to submit to your insurance company so you can be reimbursed.
Please call your insurance company to ensure you have this benefit on your plan and to inquire about your reimbursement amount for an out-of-network provider.

I accept checks, cash, HSA accounts, and I use IvyPay for credit card payments, which is HIPPA compliant.
Good Faith estimate Notice:
